• Acts as part of a multidisciplinary team designed to provide services from a holistic, trauma-informed, clinical, and values-based perspective.
• Leads the team of multi-discipline professionals in the development and recertification of treatment plans, goal setting, and overall treatment objectives for each client on caseload.
• Responsible for organizing the input from therapists, case managers, funder-assigned advocates, program specialists, behavior specialists, direct support staff, program management, medical assessments, psychological assessments, and any other input into a treatment plan with measurable goals, objectives, and outcomes.
• Provides support to program management in assessing crisis intervention when needed from a trauma-informed perspective.
• Conducts screening and assessments using validated screening tools to assess for depression, trauma, substance use, and risk/needs associated with criminal risk.
• Completes all assessments and documentation as required by RHD and others to support current and ongoing services.
• Participates in quality improvement and training activities for the purpose of compliance and program excellence.
