Position Summary
- Education: Graduate from an accredited college or university with a degree in Pharmacy required
- License: Current MT Pharmacy License required; current certification in BLS required; preceptor is preferred
- Experience: Board Certification and/or Clinical Pharmacist Practitioner designation or ability to obtain such within 12 months of hire. Equivalent combination of education and experience, knowledge, skills, and abilities will be considered
- Other: Valid state-issued driver’s license is required
- The ability to operate a computer system and apply basic computer skills
- The ability to become proficient in the use of clinic electronic medical record (EMR) programs
- The ability to communicate effectively with patients and staff
- Possess and apply the skills necessary to effectively treat patients of all age groups, regardless of the special needs of the patient
- Perform duties in a manner that is both professional and compassionate
- Apply knowledge in a manner that allows the needs of each patient to be met
- Using the patient-centered medical home team model, provide comprehensive drug therapy management (CMM) to SWMTCHC patients with the goal of improving health outcomes by optimizing drug therapy. The clinical pharmacist shall follow the Pharmacists’ Patient Care Process under a Collaborative Practice Agreement (CPA) which includes:
- Assessing the patient: basic vital signs, visual inspection for medication-related side effects
- Interviewing patients to obtain information regarding drug use and history
- Screening for drug interactions, other adverse effects, or contraindications
- Adjusting medications based on current guidelines
- Providing education on drug/disease state
- Exploring opportunities to simplify the regimen and/or reduce cost
- Utilizing tools and techniques to improve patient adherence when needed
- Implementing the plan and following up
- Work with the anticoagulation team (PCP, INR Nurse, Case Manager), to provide proper education and dosing of warfarin, low molecular weight heparin, and new oral anticoagulants
- Provide medication reconciliation services to patients who are considered high risk (ie transition of care): This includes obtaining the most current records from the hospital and/or outpatient pharmacy, communicating discrepancies and concerns to PCP, updating the med list in EMR, recommending lab monitoring and future follow up
- Lead or manage multidisciplinary teams focused on quality improvement, outcomes research, and disease management projects with the goal of improving the quality of healthcare provided to SWMTCHC patients
- Maintain positive patient relationships with a strong commitment to taking into consideration patient-specific barriers in working with patients from a wide range of economic, cultural, and social backgrounds
- Display competency in utilizing electronic medical records, and documenting medication-related information accurately and concisely, according to agency policy and professional standards
- Oversee, mentor, and teach University of Montana pharmacy students during their rotation at SWMTCHC, approving all encounters containing orders or changes in drug therapy
- Provide staff education including formal in-services as requested; act as a drug therapy resource by maintaining open communication with prescribers concerning drug therapy issues; and provide oral and written drug information upon request
- Understand rules and regulations pertaining to 340B including patient eligibility; follow established guidelines for the administration of various medication assistance programs
- Operationalize innovative projects to improve the quality and cost-effectiveness of pharmacy prescribing within SWMTCHC, including utilizing established protocols such as formulary substitution protocol, medication refill protocol, etc.
- Work in the outpatient pharmacy as needed, following all retail pharmacist job functions
- Other duties as assigned

- Computer skills including pharmacy-based software programs and EMR
- Language skills: Ability to read and comprehend medical jargon, including prescriptions and progress notes
- Math skills: Ability to perform basic pharmaceutical and medical calculations
- Proven understanding of performing confidential work under HIPAA standards
- Ability to work independently without supervision
- Practices with the scope of education, training, and personal capabilities
- Follows federal, state, and local legal guidelines; follows license criteria when licensure applies
- Maintains HIPAA compliance
- Maintains OSHA compliance
- Reports any evidence of abuse or neglect to the provider or appropriate official
