Clinical Coordinator/Supportive Housing

(Warren County)

Requires travel throughout an assigned geographic area

If YES, we have a great opportunity for a Clinical Coordinator in our Supportive Housing area who is passionate, kind, and has an interest in working with individuals with Mental Illness!

As the Clinical Coordinator will:

  • C oordinate the care of consumers in community support services.
  • Provides ongoing education and support to consumers in community support services.
  • Contributes to the implementation of new ideas and knowledge that improves services and operations.
  • Consults with mental health treatment staff to support and promote successful community support services and care to achieve a high level of consumer and customer satisfaction.

POSITION DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO :

  • Provides ongoing education and support to consumers.
  • Consults with primary care physician and mental health treatment staff as required to support and promote successful services and care.
  • Provides health educational programs for consumers, staff, and family members.
  • Participates in orientation, training, and support of fellow team members.
  • Attends meetings as needed, with consumers from all community support services.
  • Participates in systems of continuous improvement for services through a dialogue with supervisor and through 1-2-1 monthly supervision.
  • Adheres to compliance with licensing, regulatory, accreditation and internal standards through inspections, peer audits, audits of clinical and administrative procedures, and documentation.
  • Utilizes the application of technology within operations and services to enhance quality, increase outcomes and lower costs.
  • Completes and/or contributes to the development, implementation, monitoring, and updating of the individual rehabilitation plan and individual rehabilitation plan review.
  • Completes IRP adjustments and modifications as needed.
  • Implements interventions heavily influenced on wellness and recovery based principles.
  • Educates on the tools and strategies to self-manage the consumer’s illness and improve their quality of life
  • Supports consumers in identifying and building upon their strengths in order to better address needs, preferences, and goals
  • Encourages an atmosphere of wellness and recovery, and emphasize individual dignity and respect;
  • Addresses the needs of consumer’s over time and across different levels of disability;
  • Applies recovery principles that can be applied to the full range of residential, engagement, intervention, treatment, rehabilitation and supportive services.
  • Teaches skill building activities that empower the consumer to achieve an improved sense of mastery over his/her illness and assist the client in regaining a meaningful, constructive sense of membership in the community.
  • Completes and/or contribute to the completion of the Comprehensive Rehabilitation Needs assessment.
  • Attend available division funded evidence-based practice trainings (i.e., IMR, WRAP, CSS, Motivational, Educational, Cognitive-Behavioral, and Engagement Strategies etc.) and implement these tools in order to provide consumers with a recovery-oriented support system that implements wellness and recovery focused principles and interventions; and ensure HR and supervisor have copies of all trainings completed.
  • Offer consumers therapeutic skills development training with the aim of promoting community integration and restoring the individual to maximum possible functional level by improving functional, social, interpersonal, problem-solving, coping, and communication skills.
  • Teaches therapeutic skills development in the physical, cognitive/intellectual, and behavioral skills related to identified goals that leads to increased competence and proficiency.

Q UALIFICATIONS AND EXPERIENCE

  • Current New Jersey Clinical License including LCSW, LAC, LPC, LSW, and LMFT who possesses a Master’s degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program.
  • Working knowledge of computer software including word processing and excel
  • Possess a valid driver’s license and an acceptable driving record.
  • Excellent verbal and written communication utilizing the English language.
  • Planning and organizational skills.
  • Ability to perform moderate mathematical functions.
  • Ability to effectively interact with all levels of personnel and the general public.
  • Ability to travel to a variety of geographical areas where community support services are offered within the agency.
