Clinical High Risk for Psychosis (CHRP) Care Manager

Knowledge/Skills/Abilities:
- Bachelor’s degree in social or behavioral science or related field – requirement
- Lived experience as a caregiver of a child with behavioral health challenges and experience successfully navigating child-serving systems – requirement
- Vehicle – requirement
- Reliable transportation

- Valid Driver’s License
- Must obtain Family Support Specialist Certification by year (1) one of hire – requirement
- Must be willing to obtain CPR certification
- Must have good communication skills and a natural ability to establish rapport with youth/young adults and families, as well as work collaboratively as a member of a team.
- Establish and maintain positive working relationships with all clients, AHS employees and community you interact with as an AHS employee
Key Job Responsibilities:
- Provide home and community-based care coordination services for youth/young adults and their families, increasing or reducing intensity based on need
- Provide support to families of youth/young adults enrolled in the program
- Conduct intake interview with members of a family to determine the areas that require support such as parenting education and skills training
- Conduct prescreening on potential clients and families as directed
- Attends trainings and seminars as appropriate
- Models and teaches effective advocacy skills, provides caregiver active support
- Assists families/caregivers to develop attainable goals that help the client function better in the home, school and community through promoting and modeling a system of core values and principles
- Assists the family in navigating various child and adult serving systems. Support the family in making the transition from services received as a child to services received as an adult
- Provide other recovery support services as needed
- Ensure coordination, referral and linkage of services to assigned consumers, including referrals for counseling, A&D treatment and other services needed
- Ensure individuals are notified of clinic appointments
- Maintain documentation, records and reports as established by the agency, SAMHSA, CHRP, etc.
- Participate in on-call rotation and be available 24/7 during their on-call week to CHRP clients who may be experiencing a mental health crisis
- Other duties may be assigned by supervisor(s) to meet the needs of the consumer and AHS.
Shift Availability:
- (1) Full-Time: M/Tu/Th: 8:00am–4:30pm; W: 8:00am-5:30pm; F 8:00am-3:30pm
