  • The Center for Family Development is seeking a Case Manager to join our Relative Caregiver Program in the Fayetteville area. The Relative Caregiver Program aims to strengthen and support families through home visitation and resource linkage to improve economic supports, education, health and well-being, and social capital for children, parents, and family caregivers who are providing care for minor children. The Case Manager will provide case coordination, assessments, client advocacy, and support with the goal of helping families achieve overall stability. The Case Manager must promote good public relations for the program and The Center for Family Development (TCFD) while maintaining family confidentiality. The Case Manager applies and implements the philosophy and mission of TCFD.

Qualifications:

  • Bachelor’s Degree in Social Work, Human Services, or related degree preferred.
  • An ability to establish trusting relationships.
  • Acceptance of individual differences.
  • Possess compassion, flexibility, and adaptability.
  • Experience and willingness to work with culturally diverse populations that are present among the target population.
  • Open to reflective practice (i.e. capacity for introspection, communicates awareness of self in relation to others, recognizes the value of supervision, etc.)
  • Strong interpersonal skills/ability to relate to people with respect for their individuality.
  • Ability to use strengths-based, person-centered, and trauma-informed approaches in work with clients, coworkers, community partners, and community members.
  • Must have valid driver’s license, automobile for fulltime business use and proof of automobile insurance.
  • Good writing and organization skills.
  • Experience with Microsoft Office.

Principle Duties and Responsibilities:

  • Maintain a limited caseload
  • Determine Program Eligibility to assure family meets program criteria
  • Conduct Assessments of the Caregiver, Parent, and Eligible Children’s needs and create care plan accordingly to address needs/goals/barriers of family
  • Maintain complete and accurate documentation, including paperwork, records, case notes, and reports as required by grant specifications
  • Conduct monthly home visits to monitor and assess family progress
  • Implement Caregiver Parenting Support Classes – individual and/or group
  • Provide Monthly Community Groups to facilitate group interactions with children and caregivers to expand family’s supportive network
  • Case Manager provides Educational Workshops once a quarter for Caregivers to educate participants on healthy lifestyle choices, share community resources, and to address and prevent impact of Adverse Childhood Experiences
  • Make referrals to additional or complementary services for clients from other community agencies and follow up, serving as a liaison between service provider and client as necessary
  • Case Manager will attend the Advisory Committee once a quarter
  • Case Manager will participate in outreach and collaboration efforts to publicize services offered by program
  • Participate in ongoing training and professional development

Job Type: Full-time

Pay: $30,000.00 - $39,900.00 per year
