Progressive Center for Independent Living, Inc. is a Center for Independent Living which, since 1996, has provided services to people with disabilities residing in Mercer and Hunterdon Counties in New Jersey. We maximize the independence of individuals with disabilities through advocacy, peer support, independent living skills training, information and referral, and deinstitutionalization. Additional services include, student transition services, community inclusive recreation, employment readiness, DDD support coordination services and disability awareness training.

GENERAL JOB DESCRIPTION

Progressive Center for Independent Living is looking for (1) Full Time Case Management Resource Specialist. This position is office based, Monday through Friday, 9am - 4:30pm (37.5 hrs./wk.) with a 30-minute daily break. Occasional evenings and weekends will be required. This position has a strong focus on Public Health initiatives. The individual is responsible for developing and implementing a health education program to address community health needs, including wellness, nutrition and emergency preparedness, which will prepare individuals with disabilities for future public health challenges.

Additionally, this individual is responsible for offering case management to a small group of consumers under our Traumatic Brain Injury Fund, provide independent living skills (ILS) training, conduct community outreach, and sharing resources to those living with disabilities. The Case Management Resource Specialist will direct his/her work complementary to the spirit and intent of the independent living philosophy.

DUTIES AND RESPONSIBILITIES

-Identify target populations and assess overall needs for future pandemics based on information from community health organizations, county and state agencies and other relevant health related data.

•Collaborate with other agencies and consult with subject matter specialists in the planning and implementation of educational workshops for our participants.

•Provide education and outreach within residential facilities, schools and in the community for individuals, families, community, and community groups around issues concerning prevention, health, wellness, nutrition, emergency preparedness and other topics related to preparing for future public health challenges.

•Provides resources to callers seeking assistance.

•Conduct outreach to local health care organizations to inform them of PCIL services and assistance available during pandemic situations.

- Oversee a caseload for those eligible to receive financial assistance under Traumatic Brain Injury by writing plans to obtain needed goods and services. Assist consumers with identifying goals, then developing and implementing individualized plans to meet the goals.

- IL Skill training includes but is not limited to the following areas: career/job development, money management, health insurance, daily living skills, household management, mobility, peer support, self and individual advocacy, assertiveness, and socialization.

- Assist consumers with housing issues, including but not limited to obtaining affordable and/or accessible housing, accessing funding to pay for accessibility modifications, communication with landlords and public housing agencies, advocacy with a landlord, and deinstitutionalization applying for SSI, SSDI and assisting with denials.

- Provide individual advocacy to complement consumer self-advocacy, when needed and appropriate.

- Facilitate timely completion of Independent Living Plans or goals, providing services according to those goals.

- Maintain appropriate interpersonal relationships with consumers, staff and peers.

- Conduct systems advocacy as opportunities arises or are requested by supervisor.

- Assist with the provision of peer counseling services, which includes training of new peer counselors and participate in implementing, refining and maintaining the Peer Counseling program in accordance with the philosophy of independent living.

- Conduct intake interviews for Access Link.

- Offer articles and information to our quarterly newsletter and social media outlets.

Required Qualifications

- Fluency in English speech and writing.

- Minimum of Associates degree.

- Demonstrated commitment to the philosophy of independent living.

- Familiarity with a range of independent living skills and ability to teach others such skills.

- Demonstrated ability to problem solve with creative, individualized approaches.

- Demonstrated interpersonal skills. Knowledge of and ability to work with community resources.

- Demonstrated verbal and written skills sufficient to use IL skills training materials, and report on individuals’ progress.

- Creative planner with good organization and documentation skills.

- Knowledge and experience using a personal computer.

- Willingness to work flexible hours, including some evenings and weekends for outreach activities.

- Qualified person with a disability, preferred.

- Travel within Mercer County.

- Experience developing and implementing individualized goal plans and coordinating work with others.

- Documenting services and community activities in a database.

- Proficient in Microsoft Word and experience using a database to document services.
