Job description
Description: The Case Manager performs both utilization review and discharge planning. They ensure patients are admitted and transitioned to an appropriate level of care, have an effective plan of care and are receiving prescribed treatment. The Case Manager will also be an advocate for services and plans needed during and after their stay at the hospital.
