HCHC is recruiting for a Case Management Assistant. This position will support the Behavioral Health Team in assisting patients in accessing mental health and substance use services at HCHC.

The case management assistant will support the Behavioral Health program by providing phone contacts and reminders to providers and patients per protocol. The case management assistant will assist in scheduling and confirming appointments for the Behavioral Health providers as well as managing referrals made to the program.

IV. Education/License/Certification:

1. High school diploma or equivalent is required. College degree is preferred.

V. Skills and experience

1. Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required

2. Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences

3. Experience working in a fast-paced environment is a plus.

4. Fluent in Bilingual English and Cantonese/Mandarin are required.

5. Related work experience is strongly preferred.

Job Type: Full-time

Pay: $20.00 - $28.00 per hour
