Position Summary:
Provides hospital case management services, including but not limited to utilization management and discharge planning, to assigned patients to assure efficient, cost-effective patient progression from admission through discharge. Coordinates with social services for optimal post-acute care services. Utilizes established clinical guidelines to support appropriate level of care and medical necessity throughout the hospitalization, and ensure authorization for services provided from third party payers. Works with the multidisciplinary team including physicians, staff and payers to ensure efficient delivery of quality, cost-effective care. Ensures compliance with state and federal regulatory guidelines and adheres to Canyon Vista Medical Center Utilization Management Plan. Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions. Complies with legal, hospital and departmental policies, procedures and position requirements.
Minimum Qualifications:
Associates Degree in Nursing or higher, required.
One year experience in Case Management/Utilization Management, preferred with five years experience in an acute care setting.
Active Arizona or Compact RN license required.
Basic computer skills, i.e., MS Word, Excel, Outlook email and basic data entry, required.
Knowledge of office equipment usage such as telephone skills, faxing and copying preferred.
- Basic knowledge of Joint Commission standards, government and private insurance benefits, e.g. Medicare, Medicaid, DRG’s, managed care and working knowledge of clinical review guidelines for medical necessity.
Knowledge of discharge planning and community resources preferred.
