Care Team Specialist/Organizer

Giving Home Health Care is eager to find motivated, caring individuals to be an advocate for our clients who have previously worked for the Department of Energy.

Are you great with time management, organization, spreadsheets, and planning? Giving Home Health Care is looking for candidates to join our team. As a Care Team Specialist (CTS), the role of the Organizer assists teams with streamlining and keeping all information up to date to assure processes and tasks are completed within a certain time frame keeping the team afloat. The CTS Organizer will use efficient tracking and monitor all activity in a specific computer-based system to make sure all due dates are being met, tasks are being followed up on, and that nothing is overlooked. The Organizer also checks for auth period/staffing hours, and updates trackers and systems as needed. The Organizer also monitors all roles for any overflow or backlog. The CTS Organizer will save the paperwork into the correct folders, compile all paperwork together and forward it to the team lead for submission. The role will require regular outreach to field staff.

Candidate must have strong time management, organizational skills, ability to prioritize, multitask, with minimal supervision in a collaborative and positive team environment. A collaborative team player who values the success of the whole team in addition to their own success and client satisfaction.
