Shift: 5 Days a week, flexible times, on call as required.

Care Manager Summary:
The primary role of a Care Manager is to advocate for the needs of our Clients. The Care Manager will be responsible for client assessment, reassessment, and care planning, ensuring individualized Care Plans. This position requires a high degree of quality and accuracy along with a minimum of 3 years of experience as a Care Manager. In order to have success in this position you will need to be driven, proactive, self-sufficient, and have great time management skills. The requirements of this position are well defined and but the application is at the discretion of the Care Manager and Owner. Being a Care Manager requires working within the State’s and the Organization’s requirements while demonstrating of the values of integrity, quality and caution. The Care Manager will have input into our Organization’s SOP’s; new ideas and concepts are welcome. Direct access to management will promote success in this role. This role requires making decisions that support company guidelines regarding client and employee safety, and compliance with State regulations.

Care Manager Essential Functions:
  • Demonstrates passion and commitment to excellence.
  • Applies well understood principals of Care Management and builds Client Care Plans.
  • Schedules and performs supervisory visits with Clients and Caregivers to review existing Care Plans and evaluate performance.
  • Strong ability to capture appropriate information through verbal discussion and create Care Plans.
  • Possess a calm and comfortable presence in Clients homes.
  • Understand and adapt appropriately to issues related to communication, cognition, or other barriers.
  • Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner.
  • Identify conflicts or inefficiencies in Client care and make judgement calls as to the best path forward.
  • Oversee the management of Client Care Plans.
  • Performs on-call coordinator duties as needed.
  • Perform new Caregiver trainings, orientation, and disciplinary actions.
  • Ability to multitask in a high-functioning office environment.
  • Serves as a team player within an office environment.
  • Other duties as assigned by the Owner.

VERBAL COMMUNICATION
  • Answers telephone in an upbeat, professional manner.
  • Communicates continually with Caregivers and Clients to evaluate Client and Caregiver satisfaction.

WRITTEN COMMUNICATION
  • Ability to write Care Plans.
  • Ability to message Clients and Caregivers in a friendly and professional manner.

CUSTOMER FOCUS
  • Identify changes to Client needs and adjust Care Plans as needed.
  • Ability to read verbal and non-verbal cues when interacting with Clients to understand unidentified needs.

DECISION MAKING/JUDGMENT
  • Maintains integrity in all interactions.
  • Ability to live the Right at Home vision, mission, and core values.
  • Ability to problem solve and make decisions focused on compliance and excellence.

ORGANIZATION
  • Ability to schedule and coordinate one’s own day-to-day activities while meeting our Organization’s needs.
  • Maintains accurate documentation of Client records and Caregiver certifications.

Care Manager Requirements:
  • 2-3 Years minimum experience as a Care Manager at a Home Care agency.
  • High school graduate or equivalent.
  • Knowledge of Electronic Health Record Systems, WellSky (previously called ClearCare) preferred.
  • Basic Microsoft Office and computer skills.
  • Excellent telephone and interpersonal skills.
  • Ability to work independently, demonstrating sound judgment.
  • Read, write, speak and understand English as needed for the job.
  • Be available as required for on-call duty outside of normal office hours.
  • Comfortable with conducting home visits and commuting within the Boulder, Longmont, and Westminster service area.
  • Vaccination for COVID-19, willingness to get COVID-19 vaccine, or written letter of exemption.
  • LPN or RN license a plus.
  • Ability to pass all required background checks.
