Job description
The successful candidate will work out of the Council’s Xenia office and will provide information, referral and assistance related to community resources, services and support, and other consumer-related issues for Greene County seniors and caregivers. The successful candidate will also conduct in-home assessments, set-up care plans, follow-up with provider agencies, and provide ongoing care management to Greene County seniors and their caregivers, and . In addition, the successful candidate will work with fellow care managers to facilitate caregiver support groups and/or a support group for those with Parkinson's disease. Candidates must be a licensed social worker and have a minimum of 3 years of experience working with the elderly and family caregivers, including a strong knowledge base of in-home and community based services. Excellent organizational, written and verbal skills are required.

Pay: $56,000.00 - $65,000.00 per year
