Care Coordinator, SOARing in Mississippi
Job description
Provides case management and trauma-informed crisis intervention, advocacy, outreach, emotional support, and care coordination to adult and minor human trafficking survivors. In addition, they will provide informational presentations and outreach within the community.
Primary Duties and Responsibilities
1. Conduct crisis response in person to survivors of human trafficking
2. Provide advocacy for victims with law enforcement, courts, medical staff, and other service providers
3. Conduct assessments of victims’ immediate threat of harm
4. Identify relevant resources and services in the community, provide appropriate information, practical assistance, and referrals, and facilitate connections with appropriate service providers.
5. Identify and work to resolve challenges or barriers related to health equity for racial, ethnic, and linguistic minorities, people with disabilities, and sexual and gender minorities
6. Secure client transportation as needed to community locations
7. Accompany clients to court/legal proceedings as needed
8. Serve as liaison with community agencies and referral sources to better serve clients
9. Provide outreach and informational presentations to community partners, including healthcare providers, law enforcement, educational institutions, non-profit organization, and other social service agencies.
10. Work closely and collaboratively with agency staff at the Gulf Coast Center for Nonviolence
11. Maintain comprehensive and organized files utilizing the automated case management system
12. Adhere to the agency’s confidentiality policies, maintaining a high-level standard of confidentiality
13. Compile and accurately report all applicable program statistics on time and as directed
14. Report any known or suspected child abuse to the immediate supervisor
15. Attend and participate in agency and grant-related staff meetings and trainings
16. Perform all other duties and/or participate in special projects as assigned
- Ability to work diplomatically to gain and maintain professional relationships with various community partners and contacts
- Ability to be empathetic and skilled in active listening
- Ability to navigate the criminal justice system
- Ability to work with minimal supervision, as well as in a team environment
- Ability to be on call for emergency referrals 24/7
- Ability to express complex information and ideas both verbally and in writing.
- Ability to present with a strong aptitude for building relationships
- Ability to supervise student interns.
- Ability to work with a diverse population in a culturally competent manner.
- Knowledge of community resources.
- Ability to work sensitively with survivors and individuals in crisis.
- Proficiency with PC systems and MS Office Suite.
- Ability to travel across the U.S. Some overnights required.
- Fluent in written and spoken English.
Preferred Qualifications
Experience with data entry and analysis
Certificates, Licenses and Registrations:
Minimum of LSW or equivalent
Supervisory Responsibilities:
Social work student interns (BSW and MSW) – (Up to 2)
