Responsibilities: As a Bereavement Coordinator at Snowline Hospice, you will be responsible for providing compassionate and comprehensive care to our patients and their families. Your duties will include:
- Assist the Bereavement Services Manager in planning, designing, implementing, facilitating, and managing bereavement programs, support groups and related activities
- Ensure initial bereavement contact with hospice family survivors
- Coordinate, plan, facilitate and participate in the Healing Altogether (HAT) program
- Provide grief information and support through letters, educational material, individual contact support groups and counseling sessions
- Respond to inquiries and request from community members for information and referrals for grief support and other related issues
Your Experience and Education:
- Master’s degree in social work or a counseling related field from an accredited graduate school
- At least two (2) years postgraduate experience, preferably in the health care and grief
- Excellent communication, interpersonal and counseling skills
- Demonstrates skills in assisting individuals/families who have experienced loss and grief
- Skills and experience in designing and facilitating small groups
- Valid CA driver’s license required. A reliable vehicle that is insured in accordance with state requirements or willingness to participate in our company car program
- Demonstrated ability to efficiently use a computer required
- Prior electronic medical record (EMR) experience preferred
- Occasional evening and weekend availability may be necessary upon occasion
- Commitment to providing compassionate and patient-centered care
- Knowledge of trauma-informed care principles preferred
