I. Position Summary:
This position has strategic and operational oversight of Hospital Based Case Management. It is designed to provide seamless transitions across the continuum of care for MHS patients. This role requires a highly collaborative, interdisciplinary approach to patient centered care coordination focused on safe, reliable, and timely patient discharge. The position will report directly to the SVP of Revenue Cycle.
II. Accountabilities and Essential Job Functions:
  • Oversees compliance with regulatory agency requirements within Care Transition departments which includes discharge planning, care coordination, and social work,.
  • Empowers the coordination of clinical care management activities, transitional planning and denial management by setting organizationally aligned strategic objectives and goals.
  • Works closely with care management directors to achieve high employee engagement and patient experience while remaining fiscally responsible.
  • Works closely with hospital administration, physician groups, and clinical leaders to manage length of stay, utilization of services, denials, and readmissions.
  • Participates in hospital/system initiatives to formulate and implement process improvements which mitigate lower length of stay and reduce readmissions.
  • Ensures department meets monthly budget and provides information to the SVP Revenue Cycle as needed for variance analysis.
  • Provides direct supervision for Case Management leaders including evaluation, performance, hiring, education, and training.
  • Works closely with hospital staff to identify opportunities for improvement, facilitates processes/procedure changes, and provides oversight for action plans and follow up.
  • Assume a lead role for innovation, knowledge sharing and leading practice identification within MHS and among peer groups for state and grant funded project activities.
  • Practice and adhere to the “Code of Conduct” philosophy and “Mission and Value Statement”
  • Other duties as assigned
III. Education Required:
  • Bachelor’s Degree in nursing or Social Work required
  • Master’s Degree in health care field required
  • Accredited Case Management (ACM) and/or case management certification (CMC/CCM) preferred
IV. Experience Required:
  • Minimum of five years clinical experience preferred
  • Minimum of five years health care management experience required

  • Project Management - assesses work activities and allocates resources appropriately
  • Start-Up Operations - understands complexities and needs to start-up, build and maintain a new business
