GENERAL SUMMARY/BASIC PURPOSE OF JOB:

The ambulatory clinic nurse works collaboratively as an active member of the inter professional team to provide therapeutic patient and family centered care. The ambulatory clinic nurse systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The ambulatory clinic nurse reflects the mission, vision and values of Sinai Health System, adheres to the organization’s Code of Conduct and Corporate Compliance Policies and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. The ambulatory clinic nurse considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The ambulatory clinic nurse participates in continuous quality improvement initiatives and assumes responsibility for their professional development


MINIMUM EDUCATION:

  • Graduate of an accredited school of nursing (required) Bachelor of Nursing degree (BSN) (preferred)

MINIMUM WORK EXPERIENCE:

  • Three years of experience (5 years preferred) working with chronic conditions, evidence-based medicine, care coordination, care management, and psychosocial and behavioral factors affecting health outcomes. Preferred work in an ambulatory care setting.

KNOWLEDGE & SKILLS:

  • Analytical ability is required
  • Communication skills are required in order to assist and teach
  • Ability to interact effectively with others in difficult situations
  • Ability to organize and be self-motivated
  • Experience with ambulatory operations and ambulatory nursing care
  • Experience with process improvement and quality metrics
  • Knowledge and experience documenting patient care in an electronic medical record
  • Excellent verbal, written and presentation skills
  • Ability to make knowledgeable clinical assessments and judgments
  • Ability to educate staff and patients
  • Ability to provide support to the members of the care team (teamwork)
  • Must possess initiative and ability to work independently
  • Ability to maintain confidentiality of medical records
  • Basic computer skills, knowledge of EPIC or EMR preferred
  • REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:

  • Current license as a Registered Nurse State of Illinois Professional Nurse.
  • CPR/BLS Certification
