Experts in pediatric critical care, rehabilitation, and neonatal and perinatal medicine provide care for thousands of children every year at Children’s Hospital of Michigan, Children’s Hospital of Michigan - Troy and six ambulatory sites.

Job Description:

Under limited direction and according to established policies and procedures, acts as a member of the ambulatory care team by assessing, developing, implementing and monitoring a complete plan of care through the interdisciplinary team process in conjunction with patient and family.

  • Facilitates patient care in the ambulatory setting, inpatient setting and in the home.
  • Provides support, direction and/or supervision to other members of the health care team.
  • Provides consultative services within a clinical area of specialization.
  • Serves as a patient advocate and provides emotional support and home care instructions for patient and family. Assists the patient and family in adjusting the lifestyle to a disease state.
  • Communicates with patient, family caregivers, medical team, and third party payers.
  • Formulates, implements and evaluates educational strategies for patient and family.
  • Assists the family in coordinating/scheduling specialty visits, and works with third party payers to obtain authorization for procedures, medications infusion therapy and hospitalizations.
  • Analyzes care coordination outcome data to identify issues and make recommendations.
  • Maintains current knowledge of health care techniques and practices by participating in educational programs. Serves as a resource to inpatient nursing related to specialty.


Qualifications:


1. Graduate from an accredited school of Nursing. Bachelor s degree in Nursing (BSN) or other health-related field, or equivalent combination of education and or related experience.

2. Licensed to practice as a Registered Nurse (RN) in the State of Michigan.

3.Three years to five years nursing experience, including acute care experience.

4.Experience in the area of specialization strongly preferred.

5. Specialty certification by professional organization preferred.
