Job Title: Advanced Practitioner Registered Nurse
Reports to: Director of Nursing / Medical Director
FLSA Classification: Exempt
Job Summary
Provide comprehensive and efficient consultative patient care under protocol with physician staff. Utilize clinical expertise and participate in management decision making while promoting maximization of quality of life for the patients and caregivers.
An essential part of this job is to facilitate the singular opportunities for finding meaning, personal growth, and reconciling actions and relationships that a person’s dying presents, both for them and for their family members.
Essential Job Functions/Responsibilities - Responsibilities of the Advanced Practitioner Registered Nurse include, but are not limited to the responsibilities of the Hospice Registered Nurse and the following:
- Provide consultative palliative care through hospital, nursing home, assisted living facility, home, or clinic visits as appropriate and as designated by the Medical Director
- Provide hospice care, including Face to Face visits, as directed at home, Hospice Hale, nursing home, and assisted living facility
- Work directly under protocol with the Medical Director
- Provide clinical expertise and participate in interdisciplinary team decisions regarding diagnosis and medical management of patient/family needs, assuring that the highest quality of medical care is provided, and that continuity and comprehensiveness of care is maintained in a cost-effective manner
- Consult with patients’ primary, attending, and consulting physicians regarding integration of patients’ hospice and palliative care with their current plan of care
- Report regularly to Medical Director regarding clinical care provided
- Evaluate patients for eligibility for hospice care according to current guidelines and make referrals as appropriate
- Utilize practice opportunities to provide expert advanced nursing care to patients, to evaluate nursing practice, and to provide clinical learning activities for internal staff and the staff of our partners
- Influence nursing practice by participating in patient care advocacy issues directed toward improving practice and the quality of patient care
- Act as a role model for professional practice by integrating all components of the professional role: clinical practice, consultation, collaboration, leadership, education, and research/evaluation
- Collaborate with all disciplines within the agency and community partners to develop effective patient care programs
- Participate in after-hour and weekend coverage for hospice and palliative care patients cared for by agency staff
- Maintain defined patient care data for clinical activities
- Maintain accurate record of administrative, non-billable activities
- Utilize appropriate CPT and ICD-9 codes that are supported by medical documentation.
- Complete timely medical documentation of medical information on all patients seen by nurse practitioner using the electronic health record, as available.
- Promote the company’s programs to nursing homes, ALFs, insurance companies, hospitals, and other referral sources.
- Actively participate in ongoing education about our company, hospice care, and palliative care services to our community partners, referring providers, and general public
- Attend palliative care and hospice staff meetings regularly
- Represent the company at appropriate local, state, and national levels.
- Evaluate patient care outcomes and propose solutions
- Participate in appropriate Hospice Maui-wide committees
- Participate in development of clinical policy and procedures to improve quality of care delivery
- Assist in the development, implementation, and evaluation of standards of practice within the palliative care and hospice specialty
- Other duties as assigned by Medical Director*.
Position Qualifications (Knowledge, Skills, and Abilities)
- Master’s degree in nursing
- Possesses and maintains a current Hawaii APRN license
- Possess and maintain APRN certification recognized by CMS for Medicare billing
- Currently have or obtain and maintain CHPN certification
- Knowledge and experience in palliative medical practice and symptom management
- Advanced clinical skills which allow thorough patient assessments and management for any patient with advanced chronic disease as well as for those near end of life
- Basic computer skills including use of email and word processing (Microsoft Outlook and Office or equivalent programs)
- Strong interpersonal skills, communication, collaboration, and conflict resolution
- Commitment to the research process and application to practice
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
- Ability to work independently exercising discretion and independent judgment
- Ability to work in collaboration with other disciplines within the organization
- Possess and maintain CPR certification
- Possesses and maintains a current driver’s license and have reliable transportation to be able to meet visit schedule

While performing the duties of this job,
- Must be able to physically access any home in any are of the Hospice Maui service area, i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air-conditioning
- The employee is frequently required to talk or hear
- The employee is frequently required to stand; walk; use hands to finger, handle, or feel and reach with hands and arms
- The employee must occasionally lift and/or move up to 100 pounds
- Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus
- Office work would require the ability to lift files, open file cabinets, stand and bend
- The position requires intermittent physical activity including standing, walking, bending, kneeling, stooping, and crouching as well as lifting and supporting patients
Work Environment
- Work will involve an office setting with routine use of office equipment such as computers, phones, photocopiers, fax machines, and filing cabinets and clinical settings at patient homes, community facilities and in-patient facilities.
Travel
- Travel is intermittent and primarily local during the business day
Continuing Education Requirements:
- Assume responsibility as a self-directed professional for own continuing education. Identify needs or seek direction in identification of needs to maintain expertise and keep current with health care trends and nursing practices.
- Participates in appropriate continuing education as required.
- Attends mandatory educational programs.
