JOB ANNOUNCEMENT:

FULL TIME TRANSITION TO INDEPENDENT LIVING COACH

Position Description

Smith Community Mental Health is seeking a Transition to Independent Living Coach (ILC) who will employ a strengths based approach to support youth identified with EBD (Emotional and Behavioral Disorders) to prepare to transition from adolescence into adulthood. The ILC will provide one-on-one adult support to these emergent adults as they identify and access critical basic needs and services related to their successful transition to independent living. Services are provided in the community including home, school and work.

Primary Responsibilities

  • Facilitate the development of both formal and informal supports for each assigned youth participants. The ILC will engage young adults with programs, services and opportunities within the following four domains: personal growth, physical and mental health, financial health and social well-being and engagement.
  • Coach youth as they develop a vision of what they’d like to achieve within the four domain areas listed above. Through the co-creation of an transition plan that supports the youth’s understanding and practice of behaviors and skills that can develop confidence, promote independence and establish permanency.
  • Connect young adults with opportunities to discover their strengths and interests and to explore pathways to education and employment. Provide guidance related to employment, job readiness (resume writing, job leads, interviewing preparation, etc.) as well as high school graduation, GED completion and post-secondary pursuits (scholarships, applications, etc.).
  • Provide youth support as they identify access and navigate systems designed to support their transition into living independently including obtaining medical coverage as well as forms of public assistance.
  • Provide basic financial information to youth about earning money, spending it wisely, saving and investing so that they may maintain the sufficient economic resources to meet their daily needs.

The ideal candidate must have a Bachelor’s degree in Social Work, Sociology, Psychology, Guidance and Counseling, Child Development, or closely related human service field from an accredited college or university, must past a level II background screen and participated in all required training. They will have a minimum of two years of related experience, or equivalent combination of education and experience, and experience working with the following sectors: school districts, juvenile justice, mental health, law enforcement and human service agencies, preferred.
