QUALIFICATIONS:1. Holds a Bachelors Degree in Social Work or a Bachelor’s Degree in a Human Services field including, but, not limited to Sociology, Gerontology, Special Education, Rehabilitation Counseling, and Psychology.
2. One year of supervised Social Work experience in a Health Care setting working directly with individuals.
PHYSICAL REQUIREMENTS:
1. Must be able to work extended hours as needed for the facility.
2. Must be able to stoop, kneel, crouch, reach, push, pull, climb, lift and carry heavy objects.
3. Must be able to sit for extended periods of time doing charting and paperwork.
RESPONSIBILITIES:
1. To develop and maintain supportive relationships with residents and families on assigned units through regular visits counseling and assistance.
2. To identify and assess the psycho-social needs of residents on assigned units and to take appropriate steps to meet those needs.
3. To obtain a Social History and yearly Social History Update.
4. To be a member of the Interdisciplinary Conference Team involving residents of assigned units.
5. To complete appropriate assistance and support to resident's family members.
6. To interpret the Residents' Bill of Rights to families, residents and staff.
7. To provide assistance on an on-call basis on assigned weekends and evenings.
8. To be available on an on-call basis on assigned weekends and evenings.
9. To record psycho-social data on a regular basis and to retain pertinent, confidential date in resident's personal file in social worker's office.
10. To arrange and chair family conferences as needed.
11. To provide a private atmosphere for the delivery of social work services where confidentiality is respected and insured.
12. To act as resident advocate to facilitate resident and/or family grievances and to facilitate needed communication with other disciplines on behalf of the residents.
13. To be a member of the Long Term Care Social Workers Organization.
14. To work in Coordination with all other departments to meet each resident's physical, social and emotional needs.
15. To coordinate any psychiatric referrals for residents on assigned units.
16. To act as Discharge Planning Coordinator.
17. To play an active role in the Core Curriculum Program and Inservice Education Programs.
18. T0 conduct the yearly Bill of Rights Inservice.
19. To coordinate all room and specialty unit transfers.
20. To obtain Bed Reservations and arrange resident Leave of Absence as needed.
21. To interpret resident needs and behaviors and extend professional intervention to all levels of staff suggesting positive approaches, such as alternates to the use of restraints and psychotropic drugs.
22. To use and work in cooperation with appropriate community resources, coordinating and referring residents for needed and requested services and outside resources not available in the facility.
23. To assist residents and designated representatives in completing forms and clarifying information regarding Advanced Directives.
24. To provide bereavement counseling for roommates, families and other affected individuals.
25. To complete all necessary Screens in the absence of the Admissions Coordinator.
26. To coordinate admissions in the absence of the Admission Coordinator.
To assume additional responsibilities as assigned by the Administrator. Responsible to the Administrator.
