Q Hospice is currently seeking a PRN Social Worker. Working collaboratively with other members of the hospice care team, you will work with the interdisciplinary team in the delivery of all hospice care to patients and families.

JOB REQUIREMENTS

• Master's degree in Social Work from an accredited school
• Candidate must have good clinical skills and be able to work independently and effectively communicate with an interdisciplinary team.
• Knowledge of family and group treatment, community resources, and the dynamics of terminal illness, grief, and bereavement are essential.
• History working with Federal and state programs
• Broad understanding of community resources

Responsibilities:

• Develops and maintains therapeutic relationships.
• Plans care and intervenes effectively.
• Provides effective pain/symptom assessment and management.
• Promotes patient safety.
• Facilitates smooth transition between care environments.
• Provides effective patient/family/caregiver/caregiver teaching.
• Supports the death/dying process.
• Manages end-of-life ethical issues effectively.
• Collaborates in providing patient care.
• Creates timely and accurate documentation.
• Utilizes resources effectively and efficiently.
• Adheres to scope of practice
• Maintains and enhances professional skills.
• Adheres to high standards of personal and professional conduct.
