Putnam Ridge is a 160 bed Skilled Nursing and Post-acute Rehabilitation facility, located on beautiful Mount Ebo in Brewster, NY. We are looking for a qualified full-time Social Worker to organize and plan Social Service Programs for residents and patients based on their individualized needs within the facility.

• The Social Worker will serve as resident advocate, supporting resident’s rights in the facility and assisting staff, residents, and their families with rights interpretation
• Facilitate the adjustment of resident admission to the Facility
• The Social Worker will serve as liaison between the residents, their families, and the facility staff, as necessary and appropriate in the understanding of residents’ psychosocial needs and motivations
• Prepare the resident’s social/psychosocial history, assessment, and the Social Service Care Plan
• Assist the resident and their family in understanding and accepting the resident’s strengths, limitations, and understanding the plan of care so that they may be optimally supportive of the plan and participate in plan revision as necessary and appropriate
• Assess discharge potential and intentions of each resident, and specify any plans on the resident’s written care plan in the medical record
• Review and update psychosocial assessment, care plan, and discharge plan
• Record updated assessments/plans in the residents’ medical record
• Demonstrate administrative/clinical expertise consistent with standards of the facility as a Social Worker
• Consults with the Director of Social Services and works as part of an interdisciplinary team to enhance the social and emotional well being of the resident.

Requirements:

• A minimum of a Bachelor’s degree in Social Work or a related field
• Must have a sincere desire to work with the elderly, chronically ill, and disabled
• Experience in the social services field as a Social Worker or related title, preferably in a nursing facility or related healthcare facility/agency
• Pleasant personality and ability to work with residents, families, and staff
• Exemplary customer service, communication, and organization skills
