Full Time Social Worker

As a member of the Health Care Services team, the Social Worker focuses on the emotional and pyscho-social wellness needs of all residents. The Social Worker's role involves acting as a liaison between the residents, family and staff; improving communication and relationships; and acting as an advocate for residents. The Social Worker is essential to providing a "holistic" level of care for the residents.

Social Worker position highlights:

• Facilitates resident preadmission assessment, assists residents and families during admissions, transfers, and discharges.

· Educates staff members relative to the needs of residents, participates in meetings, and participates as an active member of the interdisciplinary team.

· Coordinates available resources as needed to address the needs of the residents.

· Promotes and supports the socialization, spiritual and emotional needs of residents through collaboration with the resident, their families and other personnel.

· Assists with and educates residents on entitlement programs and available resources that may be of benefit to the resident.

Requirements:

· Bachelor’s degree in Social Services or other related field

· One year social work experience in health care setting working directly with individuals.

· Experience working with geriatric persons or working with dementia residents a plus

· Knowledge of HIPPA rules and regulations and of regulations and guidelines governing long-term care

· Ability to coordinate numerous tasks and complete those tasks within given deadlines
