Carmen [kidney] HEALTH is hiring an experienced social worker to become the renal social worker guru for our operations focusing on both CKD and ESRD populations. This role will be essential in defining, establishing, and growing the social worker program for San Antonio and rural populations.

This role will be responsible for leading and coordinating the Carmen [kidney] HEALTH social work program related to Carmen's mission and values.
The role will be in continuous contact with the CNO and CEO. Part time/ contract with the potential for full time.

Key Responsibilities:
- Help achieve 100% patient compliance following the Carmen Quality Index
- Promote healthy, happy, and productive patients both in the CKD and ESRD populations
- Responsible for assessing the needs of hemodialysis, home, and pre-ESRD patients
- Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments
- Counsel patients and their families on ESRD related issues including psycho social, financial, and transportation needs
- Design and distribute educational materials for the dialysis patient, conduct seminars and classes to support that population
- Assist the patient in the completion of financial forms
- Ensure transportation by supplying the patient with appropriate information and making contacts with the patient
- Participate in operational programs as required, including attending staff and in-service meetings
- Compile and develop educational materials to be used as teaching aids for patients and staff
- Perform initial assessments and reassessments in accordance with state and federal regulations
- Participate as a member of the interdisciplinary team in all required patient assessment and care planning activities
- Help maintain a productive quality assurance program and participate in quality assurance activities
- Be familiar with all emergency operational procedures
- Attend and participate in the monthly Quality Assessment and Performance Improvement (QAPI) activities
- Establish and maintain a positive relationship with coworkers, CNO, CEO, Medical Director and physicians, area nursing homes, hospitals, agencies, vendors and the community as applicable.
- Demonstrate regular and reliable attendance
- Develop and implement social work classes and support groups for renal patients.
Desired Skills & Experience:
- Must have Masters Degree from an accredited Social Work Program
- Current clinical license in the state as a Social Worker
- Preferably +4 years experience with renal and dialysis patients
- License must be maintained as current and in good standing
- Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook)
- Would prefer bilingual in Spanish and English

Compensation: In line with market rates and adjusted to reflect for specific experiences and potential
