Social Worker Discharge Planner-Weekend Alternative

This is a weekend alternative position: Work 2 - 12-hour shifts Saturday/Sunday (7am-7:30pm) every weekend; paid for 36 and full time for benefits

Discharge Planners play a vital role in the Case Management Department. Their primary responsibility is to assist patients and their support system with planning for post-acute care. In collaboration with the interdisciplinary team, the Discharge Planner will assess the patient’s level of care, social needs, support systems and resource needs and make appropriate referrals to post-acute care. Post-acute care includes but is not limited to home care, DME, acute rehab, hospice services or skilled nursing facility. Requirements:

• 2 years previous healthcare experience preferred.
• Bachelor’s degree in Social Work, BSW, or MSW

Responsibilities:

• Assessment: Ability to effectively assess patients holistically for discharge planning needs, real and/or potential.
• Planning: Ability to use resources effectively to insure timely discharge of patients to the appropriate level of care.
• Maintains familiarity with laws, regulations, and interpretations pertinent to discharge planning.
• Computer Skills/Allscripts/Invision/Document Imaging.
• Documentation competency, skill, and accuracy in entering data into Discharge Planning software.
• In collaboration with nursing and other team members, charts ongoing assessment, planning, implementation, and evaluation of outcomes of discharge planning.
• Decision Making: Ability to make sound decisions regarding discharge planning.
• Organizational Skills: Ability to prioritize cases and complete tasks in a way to expedite discharges.
• Patient teaching activities.
• Other duties as assigned.

• Has Skills to Provides Customer Service to Patients, Team Members and Visitors

• Bachelor's Degree in related field
