Position Summary:
Responsible for the provision and coordination of services in assisting residents, significant others, and families.
Role and Responsibilities:

  • Serves as contact person for resident who need to arrange service and/or need information about the different serves offered at WM village and surrounding areas.
  • Assisting with problems of aging, illness, disability and more.
  • Coordinates appropriate internal and external resources.
  • Evaluates alternatives for care and services with residents and family.
  • Provides screening and intervention for high risk residents.
  • Assists with conflict resolution.
  • Provides individual or group counseling.
  • Represents residents psycho-social and mental health concerns on the interdisciplinary teams.
  • Serves as resident advocate.
  • Conducts comprehensive functional assessment of future residents.
  • Completes a social history, comprehensive notes, pre-admission screen, and/or MMSE as appropriate when a resident is referred.

Additional Responsibilities:

  • Acts as a consultant to Home Health resident and staff as request.
  • Facilitates level of care changes across the continuum.
  • Re-assess all resident after upon return from hospital or the health care center.
  • Serves as a member of the Ethics, CQI, Resident Care, Recollection Connection committees and represents social services on task forces as needed.
  • Arranges wellness programs.
  • Works with other departments as needed/assigned.
  • Other duties as assigned.

Education/Experience:

  • Bachelors degree in Social Work or Human Services field.
  • One year of supervised Social Work in geriatric setting working directly with individuals, or similar professional qualifications.
  • Must have excellent organizational, communication, negotiation, conflict resolution and interpersonal skills.
  • Proficient basic computer skills.

Physical Demand:

  • Lift up to 40 pounds
  • Prolong standing or walking,
  • Willing to work beyond normal working hours when needed
  • Attend and participate in continuing educational programs
  • Frequently required to walk

Job Type: Full-time

Pay: $65,000.00 - $67,000.00 per year
