SUMMARY: The Social Service Coordinator develops, assesses, plans, direct, manages and implements strategies for the delivery of social services to low-income families, elderly and disabled individuals. Social Service Coordinator coordinates, manages, monitors and collaborates with community agencies to provide case management services to participants in the Section 32 Comprehensive Homeownership, Housing Choice Voucher Family Self-Sufficiency-Sufficiency (HCV FSS) and Resident Opportunities and Self-Sufficiency (ROSS) grant programs, to enable participating families to achieve the following:

§ Increase earned income,

§ Reduce or eliminate the need for welfare assistance,

§ Make progress toward achieving economic independence and housing self-sufficiency, or

In the case of elderly or disabled residents,

§ Help improve living conditions and

§ Enable residents to age in place.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Develops, assesses, plans, direct, manages and implements strategies for the delivery of social services to low-income families, elderly and disabled individuals.
  • Coordinates, manages, monitors and collaborates with community agencies to provide case management services to participants in the Section 32 Comprehensive Homeownership, Housing Choice Voucher Family Self-Sufficiency-Sufficiency (HCV FSS) and Resident Opportunities and Self-Sufficiency (ROSS) grant programs.
  • Monitors activities of Volunteers at the One-Stop-Shop (OSS).
  • Monitors activities of Community Social Service Requirement (CSSR) participants at the OSS.
  • Conducts assessments to determine needs.
  • Recruit residents and participants for ROSS and FSS programs.
  • Recruit lenders, housing counseling agencies and other entities.
  • Monitors and evaluates program files.
  • Referral and collaboration with community agency.
  • Conduct Outreach to residents, partner agencies and employers.
  • Identifies individuals with power to allocate resources.
  • Coaches participants on individual problems; refer accordingly.
  • Conducts case conferences.
  • Maintains accurate data case files.
  • Counsels and coaches participants.
  • Meets with outside agency partners.
  • Assist in preparation of grant funding .
  • Prepares written report.
  • Coordinates Program Coordinating Committee (PCC) .
  • Calculates Monthly and Escrow credits for FSS participants.
  • Conduct Resident Council Leadership Capacity Building/Training.
  • Provides Technical Assistance to Resident Council Officers.
  • Facilitate remediation meetings for Resident Council complaints.
  • Facilitates monthly education courses (Computer, Credit Repair, Homeownership, etc.)
  • Maintains a safe work environment and exhibits safe work practices.
  • Pursue personal development of skills and knowledge necessary for the effective performance of the role. Attend training to further develop relevant knowledge, techniques and skills.
  • Performs other related duties as required.
  • Flexibility to work outside normal work hours, as required.

MEASUREMENT:

  • Timely and accurate program delivery based on needs assessment at or above 98%.
  • Effectively recruit for ROSS and FSS programs at or above 85%.
  • Follow-up on all inquiries within one business day.
  • Provide monthly training and education programs to meet needs of residents.

TEAMWORK:

  • Demonstrate and promote a spirit of cooperation and teamwork throughout the organization.
  • Continually look for ways to improve our processes and ability to serve our internal customers and residents effectively.
  • Improve personal performance on a continual basis.

CUSTOMER SERVICE:

  • Provide responsive, professional and diplomatic customer service.
  • Answer customer calls and correspondence quickly and pleasantly.
  • Communicate with internal customers and residents in a friendly, professional, and patient manner.
  • Perform all duties with the goal of building and maintaining long-term relationships.
  • Immediately address any internal customers and residents’ concerns and request the involvement of other personnel as required. Notify manager of any major concerns and the action taken to resolve.

SUPERVISORY RESPONSIBILITIES

  • No supervisory responsibilities

Pay: Up to $48,960.00 per year
