Position Title: Senior Connections Plan Coordinator
Status: Part-Time 20 hours per week/Temporary through September 2023
Reports To: Specialty Lines Supervisor
Location: Telecommute - Remote
Compensation: The starting wage for the Senior Connections Plan Coordinator position is $24.04/hr. for individuals with graduate degrees in a related field and $21.99/hr. for individuals with bachelor's degrees.

We offer help and hope to individuals and communities. Our work addresses a spectrum of needs that include mental wellness, intervention, prevention, and advocacy. We educate, train, and advocate to prevent issues of substance use, mental health challenges, and thoughts of suicide from reaching crisis levels. We are available 24/7/365 to intervene with personalized help.

Position Summary: This position aims to address the loneliness and stress of social isolation, the HOPE Lab (Helping Older People Engage) created a cognitive-behavioral model “the Model” and tools to promote social connections in later life. The Model supports older adults in recognizing that the loneliness and stress they experience in social isolation is due to both objective circumstances (e.g., disability, few social ties, barriers to socializing/physical distancing measures) and subjective perceptions (e.g., thinking they are always alone, feeling useless). This temporary position is expected to run through September 2023.

Responsibilities:

  • Demonstrate the culture of Lines for Life by modeling behavior that supports the organization’s goals and philosophies.
  • Complete required training and consultation by Pacific University and LFL.
  • Answer calls on the senior loneliness line with compassion and in accordance with the organization’s training expectations and scope.
  • In this 10-month Pilot Project, we will offer individuals who call the Senior Loneliness Line ten or more times in a month (i.e., complex needs callers), the opportunity to build a Connections Plan “the Plan” with a specialist over three free 45-minute emotional support sessions.
  • The specialist, will be working with participating individuals, will strive to shift the way these older adults think about social connection through a strengths-based, person-centered approach based on the Model for increasing feelings of connection.
  • Plan Coordinator who will be trained by the Pacific University Team to deliver Plan intervention for the duration of the Pilot Project.
  • Apply the cognitive-behavioral model of loneliness and isolation-related stress.
  • Connect remotely with older adult program participants on an initial call to describe the program and schedule first meeting.
  • Older adult participants will meet three times over the course of approximately 1 month with specialist.
  • Work with participants to identify needs to support social connectedness and set goals to change how they think, how they feel, and what they do (activities).
  • Check-in regularly with participants on progress towards goals, problem-solving to address barriers to meeting goals, and adjusting the goals as needed to support individualized needs.
  • May have to answer calls other LFL lines, we answer over 30 different crisis lines including but not limited to: the National Suicide Prevention Lifeline, YouthLine, Alcohol and drug helpline, hospital mental health crisis lines and behavioral health scheduling calls, as well as county mental health crisis lines.
  • Document inbound and outbound calls in an accurate and timely manner with relevant clinical information in an electronic health record system.
  • Communicate with team and supervisors in professional and collaborative manner.
  • Complete mandatory trainings and workshops to continue learning the latest policies and protocols and stay up to date on crisis intervention tools & techniques.
  • Perform other duties, such as administrative and organizational tasks as needed.

Required Qualifications:

  • Pass pre-employment clinical skills and technology proficiency assessment with a score of 80% or higher.
  • Able to meet Qualified Mental Health Associate requirements (https://mhacbo.org/media/QMHA.Worksheet_PDF.pdf)
  • Crisis intervention knowledge
  • Strong empathy, communication, teamwork, and interpersonal skills
  • Able to handle high levels of stress
  • Proficient in web-based computer environment and multi-line phone system
  • Understanding of and ability to apply mental health assessment, treatment and service terminology

  • Our crisis lines are operated 24/7. This position requires working some holidays and working weekends.

  • Work is primarily conducted in a remote office environment and requires the ability to sit or stand and work on a computer for long periods of time.
  • This position requires high energy, patience and controlling one’s emotions, as well as the ability to think analytically.

Remote Work: *

  • This position can be worked 100% remotely. Telecommuting requires passing a clinical evaluation, having a HIPAA compliant workspace at home, and meeting technology requirements. Supervisors provide support to telecommuters by listening in on acute calls and using an internal messaging system (Microsoft Teams) to communicate.

Quality Assurance: *

  • We want to let you know that all calls are recorded – inbound and outbound. We review them regularly to ensure safety and quality assurance and for training and supervisory purposes. What this means is that you will have calls randomly picked for review and you will receive clinical feedback on how you are doing. This happens every few weeks and is one way that we ensure professional growth of our team. Another aspect of this technology is that while you are taking calls, supervisors can listen in to provide support if a call is particularly challenging or high risk. This supports our Crisis Intervention Specialists, so they are not alone and ensures safety for our callers.
