After the initial onboarding period of up to six months, this position will work a hybrid schedule which includes a combination of working both onsite at Greenwich Drive in La Jolla and remote.

DESCRIPTION

Under the general supervision of the EDIP/UCSD at Home Program Manager, the RN Case Manager will help with case finding and support for patients in the ED to Home program. Patients voluntarily enroll in this program and ED physicians follow patients remotely to ensure the patient has what they need to stay safe at home. The RN Case Manager works with the ED Case Management team to case-find and then monitors any escalations. Typically a Home Health RN is visiting the home daily. The RN Case Manager will follow for UR and help with plan for discharge to home work order services.

MINIMUM QUALIFICATIONS

  • Must be a graduate from an accredited school of Nursing.

  • Registered Nurse (RN) license issued by the state of California.

  • Prior case management, utilization review, and discharge planning experience. Willing to consider candidates without case management experience who have three (3+) or more years of recent acute care clinical RN experience.

  • Must have knowledge of inpatient utilization review guidelines (MCG or InterQual).

  • Strong interpersonal skills.

  • Proficiency in Information Systems and computer programs including MS Office (Word, Excel, Outlook, PowerPoint, etc.).

PREFERRED QUALIFICATIONS

  • BSN/MSN

  • CCM

  • Lean Six Sigma yellow belt or above.

  • Three (3+) or more years of clinical RN experience.

  • Proven home care, managed care, or similar case management experience.

SPECIAL CONDITIONS

  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.
  • This position will be a hybrid of work from home and on-site, with more being work from home once new hire is capable with their CCM knowledge.

Pay Transparency Act

Annual Full Pay Range: $126,879 - $167,384 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $60.77 - $80.16

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
