Job description

We are seeking a Registered Nurses/ LVNs to join our team for patient visits in Houston, Texas and the surrounding areas (Baytown, Beaumont, Conroe, Humble, Pearland, League City, Richmond). You will be responsible for the assessment, diagnosis, and treatment of assigned patients. You will be responsible for SOC Oasis, Recert OASIS, ROC OASIS, Transfer OASIS and Visit Notes. Reimbursement will be based on per visit.

Responsibilities:

• Provides quality care to patients and performs technical skilled care in patient's home in compliance with nursing standards, federal, state, local government regulations, and company clinical policies
• Provides direct and individualized nursing care to assigned patients based on nursing standards
• Ensures quality and safe delivery of home health services to patients/clients and families/caregivers
• Diagnose and establish patient treatment plans
• Provides accurate and timely documentation consistent with the plan of care
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• Previous experience in nursing or other medical fields
• Ability to use WellSky/Kinnser electronic healthcare documentation software
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

Job Types: Part-time, Contract, PRN, Per diem

Pay: $30.00 - $120.00 per hour
