Position Description:
Responsible for the delivery and evaluation of medical nutrition therapy services to agency patients. This is a per diem role supporting Oneida County
Performance Responsibilities and Standards:
- Provide consultation for policy and program development and operation and evaluation of nutrition services (visits to be conducted in person primarily or via Telehealth as needed).
- Serve as a consultant to the agency in the application of nutrition knowledge.
- Provide direct service to patients and families through home visits. Assess nutrition status and food practices; assist in planning for the provision of food and fluids appropriate for the individual's physical and environmental conditions. Provide nutrition education and counseling to meet nutritional needs.
- Provide appropriate educational materials for staff and participate in in-service training for staff.
- Demonstrate knowledge of and ensure compliance with agency policies and procedures.
- Demonstrate knowledge of, and ensure compliance with, federal, state, and local regulations governing home care.
- Complete nutrition care documentation in a timely manner. Communicate with staff / Physicians regarding patient nutritional needs.
- Complete nutritional assessments for enteral feedings.
Requirements:
- Bachelor's degree with a major in foods and nutrition from a college or university approved by the Educational Department. Registered dietitian status with the American Dietetic Association; New York State Certified Dietitian/Nutritionist. Maintain registration status through approved continuing education activities.
- Two to three years of professional experience in a health agency which includes counseling patients in normal and medical nutrition therapy.
- Current New York State driver’s license and automobile insurance. Ability to drive in varied weather conditions throughout the year.
Physical Requirements:
- Speech/visual/hearing ability sufficient to comprehend and express written and verbal communications.
- Ability to lift up to 10 pounds, kneeling, standing, walking, and stairs.
- Bloodborne Pathogens Exposure Category Bloodborne Pathogens Exposure Category II Task is one in which there is a potential for, although not planned, contact with blood other potentially infectious materials or air borne pathogens.
