WHAT YOU'LL DO:

As a Diabetes Program Coordinator, you will need to be a registered professional nurse or dietitian with advanced knowledge of diabetes and clinical expertise. You will be responsible for oversight of the planning, implementation, and evaluation of the Diabetes Self-Management Education (DSME) Program, curriculum, and continuous quality improvement process, as well as establishing coordinating consistency of diabetes care in both the hospital and the outpatient setting and community services. As the Coordinator, you will ensure that the National Standards are met and maintained at all times. You will coordinate the operational goals of the Diabetes Education Program in conjunction with Health System goals. For our team members, this is more than a career — it’s a calling.

BACKGROUND:

• Bachelor’s Degree required in nursing or related field. Master’s degree preferred.
• 2 years’ experience in diabetes program management, patient education, or chronic disease care with a minimum of 2 years as a Certified Diabetes Educator (CDE).
• Current Georgia RN or LD/RD
