Job description

The Program Coordinator for Service Coordination is responsible for overseeing a team of 4 Coordinators; 2 Service Coordinators, 1 Family Support Coordinator and 1 Transition Service Coordinator. The responsibilities of these coordinators include monitoring, planning, assessing and advocating for individuals served who experience a developmental disability or an acquired brain disorder as well as intakes, transition from school to adult services and family services.

Responsibilities of this position include but are not limited to: Demonstrate an understanding of the Agency’s vision and mission, communicates it to staff and acts in accordance with it; Consistently act with honesty and integrity, while demonstrating strong commitment to quality services and the interests of individuals served; Effectively performs job requirements and develops competencies as necessary to perform the job; supervise a small team while maintaining positive working relationships, Promote problem resolution and self-management strategies with staff and the individuals served; Hire, train and supervise staff; evaluate staff performance

Education: Bachelor’s degree is desired and/or a combination of 3 years of education, training and experience in the human services field.

The right candidate must be self-motivated, detail oriented, have strong communication skills, be able to multi task in a busy environment, have coordination and supervisory skills, be flexible and be a strong team player.
