Park Pointe Healthcare & Rehabilitation Center, is a 142 bed skilled nursing and rehabilitation center, located in Morris, Illinois. We are a fast paced, high volume facility specializing in short term rehabilitation, seeking a Physical Therapist for full-time,.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Plan, develop, organize, implement, evaluate, and direct the facility’s physical therapy, as well as its programs and activities, in accordance with current rules, regulations and guidelines that govern nursing care facilities.
• Develop and maintain physical therapy standards.
• Assist in developing, implementing, and coordinating policies and procedures, resident care plans, physical therapy procedures, etc.
• Review the physical therapy requirements of each resident and assist in planning for the resident’s care.
• Participate in discharge planning, as necessary.
• Maintain treatment records, resident files, and progress notes
• Maintain an adequate liaison with families and residents
• Review requests for physical therapy and physician’s orders
• Demonstrate to residents and staff personnel, the use of medical equipment involved in physical therapy such as wheelchairs, crutches, canes, etc.
• Recommend modifications or changes in the resident’s therapy program based on evaluation of progress.
• Assist in developing safety standards for physical therapy department.

REQUIREMENTS

• Must possess a current license to practice Physical Therapy in Illinois.
• Must be able to read, write, speak, and understand the English language.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the needs of the residents.
• Must have patience, tact, and a cheerful disposition, and be willing to handle residents on the maturity level in which they are functioning.
• Must provide proof of Covid and Flu vaccination.
