Job Description:

Our home health therapists work with physicians and case managers to provide in-home rehabilitation to our patients. Together we will help our patients achieve the outlined goals and milestones on their patient care plan.

Responsibilities:

• Manage and provide skilled therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, considering individual patients' special or age-related needs
• Timely and accurate patient charting and documenting
• Interpreting assessment/diagnostic data from previous visits or surgeries
• Developing and revising the plan of care in consultation with physician and other care-team members
• Supervising therapy assistants as appropriate
• Drive to your patients’ homes throughout your assigned region on a daily basis

Qualifications:

• Texas PT license
• 2+ years’ experience as a Therapist in Home Health
• Experience in assessment of high mobility equipment preferred
• Current CPR/BLS certification
• Reliable transportation
• Valid and current Texas Driver’s License and auto liability insurance
• EMR experience (Kinsser required)

Job Types: Full-time, Part-time

Pay: $65.00 - $75.00 per hour

Schedule:

• 8 hour shift
• Day shift
• Monday to Friday
• Night shift
• Weekend availability
