Responsibilities
JobSummary:
The Population Health Pharmacy Technician will help the PHSO attributed patients thrive by assisting with identifying, tracking and reporting of patients with open medication adherence gaps. Under the direction and supervision of the Division Director of Care Management and Quality, the Population Health Pharmacy Technician assists the Population Health Pharmacists by scrubbing medication adherence reports, loading intervention opportunities in a database, and confirming patients picked up their medications.
Essential Position Expectations:
- Understand confidentiality with respect to patient/ client care; complying with all federal and state laws applicable to the confidentiality of protected health information (PHI) and electronic protected health information (EPHI); and follow HIPAA guidelines regarding readily identifiable protected health information
- Reviews medication adherence reports to remove patients who do not meet criteria for intervention
- Loads intervention opportunities into Care Management program data base, prioritizing them according to risk, and assigning them to the correct pharmacist
- Provides telephonic follow up with pharmacies to verify patients picked up medications, closing care gaps.
- Organize daily call lists in preparation for the Pharmacist
- Facilitate patients to the Pharmacist for all clinical needs
- Complete assigned tasks related to medication adherence services.
- Accessing, inputting, and retrieving information through the Care Management computer network to maintain accurate records
- Obtain refill authorizations from prescribers
- Provides medication adherence services utilizing prescribed workflows
- Participates in scheduled PSHO meetings
- Comply with established procedures, rules, and regulations
- Completes clear and concise documentation in Care Management program database
- Professionally answer and place outreach calls to patient and PHSO staff
- Handle patient issues in a calm manner and referring them to the pharmacist accordingly and notify the pharmacist of any issues
- Promoting teamwork, professional services and clear communication
- Recognizes and adheres to the scope / limitations of job requirements
- Performs other related duties as assigned
Qualifications
Qualifications and Requirements:
- High school diploma or equivalent.
- Pharmacy technician license or pharmacy technician trainee license from the Iowa State Board
- Strong communication and customer service skills
- Work a flexible schedule, including evenings and weekends
- Ability to read and transcribe pharmaceutical information
- Sit for long periods of time, stand for intermittent periods of time
- Bend, reach, climb, squat, lift, push, or pull a minimum of 40 lbs.
Experience: Previous healthcare system, health plan, or PHSO is preferred. Three to five years of pharmacy technician practice
Certifications & Licenses: Iowa Board of Pharmacy Technician registration/license; Certified Pharmacy Technician (CPhT) with PTCB
Pay Range
$16.26 - $22.36 /hour
