Duties

  • Complete Comprehensive Medication Management for Ambulatory Care Sensitive Conditions

  • Anticoagulation Management at main campus in Danville and its Community Based-Outreach Clinics (CBOCs)

  • Pain management and opioid overdose education and naloxone distribution

  • Attend team or multidisciplinary patient care rounds or meetings.

  • Collaborate prospectively with acute care providers to develop, implement, and maintain systems that promote desirable clinical outcomes of acute and chronic disease states.

  • Serve as the discharge/ transition care pharmacist from the main site in Danville, IL.

  • Complete admission and discharge notes for all patients that are transition from inpatient to outpatient setting.

  • Monitor serum levels for patients receiving drugs with narrow therapeutic indices; evaluate and adjust medication therapy.

  • Help achieve positive patient-centric outcomes and provide clinical pharmacy expertise through direct and indirect interactions with patients, providers, and interprofessional teams in assigned areas

  • Coordinate with the pharmacy and clinical staff to maintain the medical center drug formulary.

  • Work with other clinical staff in developing and establishing criteria and guidelines for patients receiving antibacterial, antiviral and/or antifungal therapy.

  • Review medication regimens of all clinic patients, and noting for provider review any findings that might require action

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in
accordance with VA Policy.
  • English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and
7407(d).
  • Education: Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of
Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
(TRANSCRIPTS REQUIRED) OR
  • Must be a graduate of a foreign pharmacy degree program that meets the educational requirement if the graduate is able to
provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes
passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language
Internet-Based Test (TOEFL iBT). (DOCUMENTATION REQUIRED)
  • Licensure: Must possess a full, current, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth
(i.e., Puerto Rico) of the United States, or in the District of Columbia. (MUST PROVIDE A COPY)

  • Grade Determinations: In addition to the Basic Requirements above, applicants must meet the following grade requirements.

  • SPECIALIZED EXPERIENCE GS-13 LEVEL: You must have at least one (1) year of experience equivalent to the next lower grade
level (GS-12) or equivalent, which is directly related to the Clinical Pharmacy Specialist position. The clinical pharmacy
specialist (CPS) functions include, but are not limited to: designing, implementing, assessing, monitoring and documenting
therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive
patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in
assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and
toxicity of medication therapy. OR
  • You must have completed 1 year Post Pharm D. ASHP accredited Residency OR
  • Must be completing a 1 year post-Pharm.D. ASHP accredited VA Residency by July 2023. In addition, you must fully
demonstrate the following KSAs:
1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
2. Expert understanding of regulatory and quality standards for their program area.
3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical
expertise.
4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of
laboratory
and other diagnostic parameters.

  • Creditable Experience: Residency and Fellowship Training. Residency and fellowship training programs in a specialized area
of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency
program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not
accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be
creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this
occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Preferred Experience: Completion of a PGY-1 Residency or Inpatient experience

The full performance level of this vacancy is GS-13.
