Overview:
20-40 hours per week

General Statement of Duties: Perform outreach and in-reach to recruit and engage men who have sex with men (MSM) and transgender individuals of color who are at high risk for HIV infection and who use drugs and alcohol. Provide risk reduction counseling and recommend appropriate screening services for those recruited, including HIV/STI/HCV screenings and substance use disorder treatment. Provide education on substance use disorder, HIV/HCV/STI, and risk reduction strategies. Provide supported referrals and health care navigation for substance use disorder, infectious disease treatment, HIV PrEP and nPEP, and primary health care. Network with community groups and agencies as needed to advance the prevention/education models that are implemented.

Responsibilities:
- Perform health navigation services, including supported referrals to address the full spectrum of individual needs and providing appointment accompaniment.
- Provide linkage to medical care, behavioral health treatment, substance use disorder treatment, PrEP, nPEP, disease intervention specialists, and other needed services.
- Work with the Director of HIV Services to develop ongoing strategies for outreach, recruitment, and service delivery.
- Screen, interview, and counsel individuals who feel they are at risk of HIV/STI/HCV infection. Provide harm reduction information and strategies for at-risk individuals.
- Collect specimens (via venous blood draw, finger stick, and/or urine specimen) for HIV/STI/HCV testing and send and/or transport specimen as appropriate to the State Laboratory.
- Timely and accurate collection of data and paperwork completion.
- Initiate and conduct follow-up activities to assess service implication and impact.
- Develop, provide, and document appropriate referrals in focused areas for clients in need of services.
- Provide transportation to persons served as needed.
- Provide and distribute all appropriate materials (condoms, bleach, and printed material) to increase the target population’s level of awareness and understanding of HIV transmission.
- Enroll clients and bystanders to participate in the Narcan Pilot Program.
- Facilitate group level interventions and health communications with approved curriculum as designated by supervisor.
- Work in conjunction with the Program Director and persons served to assess the ongoing effectiveness of the Prevention, Counseling, Screening and Referral Programs.
- Coordinate efforts with other HIV/AIDS, substance use, MSM, and transgender health programs.
- Promote the program through community outreach and information dissemination.
- Attend meetings, conferences, and training to expand knowledge of HIV disease and related topics (STD’s, Hepatitis C, substance abuse, harm reduction, overdose prevention etc.) and be current on new developments as required by supervisor or designee.
- Timely completion of all necessary paperwork and documentation.
- Participate in all staff meetings and staff supervision as required.
- Establish and maintain a good rapport and cohesive relationships with staff, clients, and other program related individuals.
- Support case management responsibilities as needed.
- Perform duties consistent with agency/program policies and procedures. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
- Perform other related work duties as needed or as required by supervisor or designee.
- Bona Fide Occupational Requirement: To be considered for this position, the applicant must self-identify as being a GBQMSM (Gay, Bisexual, Queer, or other MSM) man as well as Black and/or Latino, or self-identify as being GBQMSM with documented experience working with BIPOC GBQMSM populations
- BA/BS desired. High School Diploma or GED plus outreach/education experience strongly desired.
- Flexible schedule to accommodate evening/weekend hours required.
- Knowledge of HIV/AIDS, substance use, and Harm Reduction principles and practices.
- Knowledge of area POC MSM and transgender communities.
- POC and GLBTQ+-identified individuals strongly encouraged to apply.

- Demonstrated ability to take initiative and work in a team and individual setting.
- Approachable personality and open mindedness required.
