The GI patient navigator oversees daily operations and workflow for the GI patients including patient screening, patient scheduling, authorization, patient chart presentation for conference, and all required regulatory reporting,

Collaborate with lead oncologist overseeing program, and all supportive team members and departments to ensure excellent patient experience, timely follow up, and improved adherence,

Support efforts to increase awareness and outreach for colorectal screening,

Job Qualifications/Education/Training/Certification/Licensure:

The GI navigator directs patients through the screening, enrollment and appointment process,

Supports all efforts related to guiding patients and families through the screening process,

Schedules appointments and diagnostic studies and tracks status of patients in database,

Performs program quality assessment checks, audits, and reviews,

Performs outreach to community physicians to grow the program,

Develops and drives initiatives to eliminate barriers to the colorectal screening program,

Establishes processes and standards that ensure consistent patient scheduling including follow up diagnostic imaging exams,

Communicates with referring physicians regarding patient appropriateness for program and coordinates information with referral services,

Performs patient follow up management for patients already screened, uses database to ensure appropriate follow up appointments are scheduled and kept as recommended,

Prepares information for case presentation at tumor board, including past and present diagnostic testing results,

Distributes patient letters, provides follow-up and confirmation calls,

Responsible for database management including inputting patient screening data, patient letters, data entry input of all patient management activities, statistical reporting,

Responsible for tracking and reporting monthly all data points required by CMS and ACR,

Provide updates on program activity, statistics and recommendations for program enhancements,

Coordinates with Patient Financial Services and the Finance Department as needed,
