Navigator Outpatient Specialist
Job description
The primary responsibility of New Freedom’s Navigator Outpatient Specialist is to effectively provide members with outreach, engagement, and enrollment in aftercare services. The Navigator Outpatient Specialist works with a variety of people and groups to locate and engage community members who need behavioral health services, employment services, and restorative services which include aiding with completion of applications necessary to receive benefits and treatment. The Navigator Outpatient Specialist will work as part of the clinical team dedicated to behavioral health and substance use treatment services.
DUTIES AND RESPONSIBILITIES:
- Partner with program leadership to develop and broaden the services offered through the department.
- Provide outreach, engagement, and enrollment into Outpatient services, utilizing best practices and modalities for providing the expected level of services and support.
- Partner and work with a variety of people and groups to effectively locate and engage community members who need behavioral health services.
- Assist members with completion of the applications needed to receive benefits and any/all necessary treatment.
- Assess members’ needs and support members with their re-entry treatment planning and goal attainment.
- Partner with members of the assigned treatment team to assist the member in the attainment of their identified goals.
- Participate in and receive clinical oversight and supervision.
- Participate in team and company meetings.
- In addition, all other duties as assigned.
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
- Excellent communication, organizational, time management, and critical thinking and problem- solving skills required
- Effectively demonstrates versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm
- Effectively collaborates and works within a team, while also effectively demonstrating the ability to take initiative and work well individually
- Intermediate computer skills – MS Office (Excel, Word, Outlook, Teams), Internet, Electronic Health Record, etc.
- One (1) year of direct behavioral health service experience required
- High school diploma or GED required
- Peer Support Certification preferred
- Valid AZ driver’s license required
- Successful clearance of annual MVR required
- Successful clearance of pre-employment criminal/county/state background check
- Successful clearance of pre-employment drug and alcohol testing
- Successful clearance of annual TB PPD testing
