Angelic Paths is currently seeking a Per-Diem/PRN Medical Social Worker to join our vastly growing Hospice team in the Lackawanna County, PA and Surrounding Areas! This position is responsible for providing patient care in their home.

Responsibilities:
- To function as a resource to patients and families in accessing community services to meet their socioeconomic needs.
- To ensure quality and safe delivery of social services.
- To ensure that social services, reflecting the Agency's mission and standards, are provided in accordance with federal, state, and local standards, guidelines, and regulations.
- To demonstrate support for achieving the mission of the Agency.
Qualifications:
- Masters degree of social work accredited by the Council on Social Work Education
