Processes medical records for completion of required documentation. Maintains accurate and logical filing systems for the timely retrieval of medical information. Assists internal and external customers and patients with Medical Records inquiries promptly while displaying professional customer service.Reviews patient records to ensure compliance with legal Medical Records requirements.
Analyzes patient records to ensure compliance with local, state and federal regulations.
Identifies gaps in meeting legal requirements and enters deficiencies into the electronic medical record system.
Verifies accuracy of content including but not limited to name and visit.
Reviews patient records to ensure compliance with legal medical record expectations and department policies.
Verifies accuracy of content of record and analyzes charts accurately identifying deficiencies and entering them in to the appropriate system.
Performs Document Imaging on Inpatient and Outpatient patient records and other legal documents as needed.
Logs medical records into appropriate system and assists with record control functions throughout the department including but not limited to moving charts from one place to another, signing charts out to other departments/locations/physicians, printing facesheets, filling records, maintaining the MPI and moving completed records to permanent filing.
Scans patient charts and documents into the legal electronic medical record achieving a 95% accuracy rate with correct patient, correct visit and correct placement. Logs selected charts into appropriate system.
Processes record control functions as assigned in a timely and efficient manner meeting productivity standards no less than 95% of the time. Files completed records into the perm file in a timely, efficient and accurate manner.
Greets and provides information to patients, visitors, hospital personnel, physicians and all others when presenting to area or calling on phone.
Promptly acknowledges individuals upon entering the department and actively seeks out their needs and or concerns.
Uses good telephone etiquette and communication style to promote good public relations and interdepartmental relations.
Answers phone calls in a timely manner and identifies self by name, title and area.
Routes telephone calls and messages appropriately.
Assists customers with questions, ROI requests, copying and sending records as needed and other tasks as required.
Demonstrates a willingness to promptly meet identified needs in a pleasant and courteous manner.
Assists internal and external customers with requests for records, medical record inquires, copying records and sending records to other parties as needed
Attends and participates in staff and analysis meetings.
Makes suggestions and recommendations for improvement in office operations.
Maintains a clean, safe and functional working environment.
Participates in special projects as requested by management.
Stays engaged in department cross training, offers up suggestions and continually looks for ways to gain efficiencies and improve processes.
Attends staff and analysis meetings and participates as a team member and works effectively within office structure.
Volunteers for special projects.
Participates in cross training and offers up suggestions, improvements and looks for ways to gain efficiencies.

Education Requirements: High School Diploma or GED
