Position Summary:

The Clinical Coordinator Position is responsible for processing information necessary to maintain an efficient flow of internal and external documents pertaining to Nursing Services. They will perform duties and responsibilities in a manner consistent with core values, mission statement, corporate values and service excellence.

Job Duties (include, but not limited to the following)

- Informs clinician to make home visit of SOC to assess the home environment and initiate services.

- Monitors nursing staff productivity and assures productivity targets are met.

- Maintains and coordinates schedule of all nursing visits in patient management software.

- Assists Clinical Supervisors with Faxes and communications with physician's office.

- Informs management when nurses are not meeting frequency.

- Answer’s telephone inquiries relating to agency services and channels them appropriately.

Requirements:

- Current CNA or MA state license or home health/hospice experience.

- 1 year of medical experience.

- Current CPR/BLS Card.

- Good organizational, interpersonal and communications skills.

- Current driver's license, and auto liability insurance.

Job Type: Full-time

Pay: From $13.00 per hour
