Fourth Street Clinic (FSC) in downtown Salt Lake City has been providing those experiencing homelessness with essential healthcare services for 25 years. The Case Manager is a critical role within the Medical Street Outreach Team (STREET), assisting patients in accessing needed medical, social, educational, and other support services in both the clinic setting on-site, and off-site with patients residing in outdoor homeless encampments and among other community providers. Success as a STREET Case Manager involves effective facilitation of referral and follow-up services for medical and behavioral healthcare, insurance eligibility, transportation, care gap analysis, and improving patients’ social determinants of health.

This role is perfect for people who:

• Are confident, self-assured, and able to execute in a variety of settings and conditions, including inclement weather.
• Have familiarity with the Utah Case Management framework as established by the Department of Human Services (completed certification is preferred.)
• Are proactive advocates, driven to fulfill service interactions with clientele.
• Keep information organized and take detailed notes to meet all compliance requirements.
• Will maintain high physical mobility throughout the day (with or without accommodations), and can travel for company business around Salt Lake City and surrounding areas.
• Speak Spanish (preferred, but this is not essential.)
• Have completed their high school education with a diploma or GED equivalent. (College coursework in social work, social services, or similar degree preferred, but not required)

Last year, we provided comprehensive health care to more than 5,000 unsheltered men, women, and children through more than 20,000 visits in-person on-site, virtually, or through our Mobile Outreach team.

Pay: $20.00 - $22.00 per hour
