Aging and Family Services of Mineral County, Inc. is seeking applicants for the position of Medicaid Waiver - Case Manager Social Worker or RN. This individual will coordinate services for individuals under the Title XIX Medicaid Waiver Program. The successful applicant will be based at our Keyser, WV office but will primarily work from home with our clients in Mineral, Morgan, Hampshire, Grant, AND surrounding counties. Travel to our surrounding counties will be necessary.
The successful candidate MUST be a West Virginia licensed RN or social worker (some Other human services bachelor’s degrees may also qualify) and have the desire to provide excellent client service. Applicants should be looking for a professional, friendly, and rewarding work environment. Travel within Mineral County and bordering counties via personal vehicle is required.
This is a full-time position with a caseload of 50 - 60 clients but could be open for a part-time Case Manager. Medicaid Waiver - Case Manager Social Worker or RN
Under the supervision of the Director of Case Management has primary responsibility of overseeing the ADW Title XIX Case Management Services. Conveys office needs and/or concerns to the Executive Director.
I. QUALIFICATIONS
A. Must be a Registered Nurse (RN) or Licensed Social Worker (LSW) with a current license in the state of West Virginia. Other human services bachelor’s degrees may also qualify.
B. Must have an understanding of entire scope of program objectives and the ability to implement where required.
C. Understanding of and desire to assist the older citizen.
D. Knowledge of community resources available to provide information and referral to older persons..
E. Be able to type, file, and operate office machines.
F. Be able to operate a computer, MS Office products
G. Must have available and reliable transportation.
H. Ability to coordinate with other staff to implement programs where necessary.
I. Work as a team member with staff to fulfill agency objectives.
J. Ability to be flexible and cooperative. Due to lack of staff this position requires.
you to be able/desire to assist in new p.rograms or procedures not described in this job description.
K. Must meet all state Medicaid, Bureau of Senior Services training, and continuing education requirements for a Case Manager
II. JOB DESCRIPTION - Wavier Case Manager
A. Assists, consults and advises other staff members in day to day matters of case management operation.
B. Conducts initial evaluation assessment of client prior to requesting services from a homemaker agency.
C. Initiates and completes initial, 6-month and annual Service Coordination Plan (SCP), reviewing the plan with the homemaker RN, client and informal support person(s) -- if applicable, ensuring coordination between HM services and the SCP provided by the Case Manager (CM).
D. Documents all actions, visits, and reviews and maintains- a case file for each client. Maintains communication and works cooperatively with other community agency personnel to ensure optimal client care.
E. Assists in other duties, including CPR/First Aid training as Case Management cases build to 70+ clients.
F. Maintains contact with clients per phone or in person monthly and completes an In-Home visit and "Case Management Assessment" form every three (3) or six (6) months.
G. Completes an annual CM assessment on each client and provides proper documentation and communication of findings with Homemaker RN as deemed necessary, and as per state and federal mandates require.
H. Documents all monthly nursing- records of contacts and visits for billing purposes.

III. Work Hours/Location/Pay
A. Employee will split their time working in our office and out of their home maintaining contact with our agency through phone, e-mail and other methods. Will maintain contact with clients through phone calls and home visits as necessary. Our clients are in Mineral County and neighboring counties. Approximate travel requirement is 40%.

Maximum earned Leave Time 720 hours (90 Days)
Maximum leave cash out 50 hours
