Qualifications for MDS Nurse:
Current unencumbered State of Massachusetts RN or LPN License-
Two years of MDS and PPS experience
Care Planning experience
Responsibilities for MDS Nurse:

Conduct and coordinate the development and completion of the resident assessment -
Conduct resident assessments in accordance with current rules, regulation, and guidelines -
Implementation of resident CAA’s -
Participate in facility survey readiness and throughout survey
Work with our interdisciplinary care team min developing a comprehensive assessment and care plan for each resident.
