Position: MDS Coordinator-

Sunny Ridge Nursing and Rehab is seeking an experienced and dedicated MDS Coordinator for our skilled nursing facility. Initially, this would be a part-time position working Wednesday, Thursday, and Friday, but could become a full-time position with combined duties/ roles that may include social services assistance or floor hours.

Salary is negotiable and based on experience and skill set.

Candidates must be well versed in MDS, RAI, and Clinical Reimbursement, and Case Manager experience is a plus! This is a lead role and the ideal candidate should feel comfortable leading Medicare and Medicaid meetings. Training staff on documentation requirements and expectations. Prior experience with Point Click Care (PCC) preferred.

Essential Duties and Responsibilities:

• Assist with resident assessments/ UDAs and development of plans of care upon admission.
• Preparing for new resident admissions and discharge planning
• Assists with payer coverage matters.
• BIMS and Social Service Assessments- Part A/ Medicare Advantage Residents or assists
• Run Utilization Review Meeting, Set and Schedule MDSs for Part A/ Medicare Advantage residents
• Participate as an active member of the IDT Care Plan and Medicare Team.
• Ensures proper documentation is maintained.
• Performs quality assessment and assurance functions.
• Coordinates any record request to validate the payment.
• Participates with ED and DON in survey processes.

Qualifications:

• Current license as a Registered Nurse and able to practice in Wisconsin.
• Minimum of three years of previous geriatric nursing experience, preferable in a supervisory capacity.
• Strong knowledge of the MDS process
• Strong knowledge of Medicare A codes and guidelines in SNF.
• Possesses good organization ability and experience with computers.
• Excellent communication skills.
