3 OPEN POSITION

1 FULL-TIME LVN Position in Poway, 2 LVNs in El Cajon area Monday-Friday shifts 6am-2pm, and Monday - Friday 2pm - 10pm.

Job Summary

• The LVN is responsible to assist and care for the clients that has developmental and physical disability
• Education and Experience
• Minimum Education:High School Diploma or GED, LVN, CPR/FA, and CPI certification
• Minimum Experience:Preferred 1 year experience supporting individuals with developmental disabilities
• Minimum Driving Experience:

Must have a valid in-state driver’s license and current LVNEssential Functions

• Outline and prepare daily routine(s) for clients' (visual and active) as written in Individual Program Plan.
• Audit all medications, double check prescriptions to ensure that they match what’s on the medication administration record (MAR) and on the bottle.
• Attend to client care needs such as grooming, showering, toileting, feeding and oral hygiene Complete and update electronic client notes entries.
• Performs other related duties and assignments as required Physical Requirements.

LVN JOB DESCRIPTIONS:

1.) Audit all medications, double-check prescriptions to ensure that they match what’s on the medication administration record (MAR) and on the Bubble Pack

2.) Administer medications to individuals and follow the 7 rights for verification purposes.

3.) Monitor individual’s baseline such as normal temperature and behavior and report any significant changes to the physician and the Administrator.

4.) Attend to client care needs such as grooming, showering, toileting, feeding and oral hygiene.

5.) Perform g-tube (gastrostomy) feeding, care and maintenance to pertinent individuals. Perform Catheter care.

6.) Assist in maintaining a safe, neat and clean environment for individuals and report any possible harmful items and/or environments to administrator.

7.) Write individual progress reports and conduct ongoing assessments by following the behavior consultant’s written behavior intervention plans.

8.) Provide individualized attention to each individual client and make sure that they attain the highest physical, mental and psychosocial well-being.

9.) Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves when indicated and proper disposal of soiled materials.

10.) Complete and update client daily notes entries.

• The LVN is responsible for providing safe, compassionate, and focused nursing care to assigned clients with predictable health care needs.

PERFORMS CLEANLINESS DURING COVID.

The LVN functions in a task-oriented role under the direction of the MD/NP/RN
Provides direct patient care & implements the clients plan of care
Functions as a member of the healthcare/interdisciplinary team (MD, NP, RN, Behaviorist, PT, OT, Social Worker).
