Locations: Day Hab Residential Facilities.

3. Laurel – 222-16-134th Road, Queens, NY----- 1 LPN
4. Waterside- 10 Waterside Plaza Manhattan, NY----1 LPN
TOTAL-4 LPNS, 1 per location
SUMMARY
Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse and in accordance with policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Licensed Practical Nurse - Residential will include, but are not limited to the following:
- Refers all consults and diagnostic reports to registered nurse for review.
- Attends and transports clients to appointments as necessary.
- Attends team meetings as scheduled.
- Attends to clients’ medical conditions.
- Carries beeper and is on-call, responds to questions regarding physician and RN treatment plans.
- Keeps abreast of changes in Nursing field.
- Performs other related duties as requested.

- Ability to read, write, speak and understand English
- Good oral and written communication skills
- Good organizational skills
- Interpersonal skills necessary to interact effectively with coworkers, employees, consumers, families and outside contacts
- Must be able to maintain the strictest confidentiality
- Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices.
- Must be willing to work at various program sites
EDUCATION and/or EXPERIENCE
- Graduate of an accredited program of professional nursing
- Min of 1 year of experience as a Licensed Practical Nurse in the delivery of community-based health services to individuals with developmental disabilities.
LICENSE/CERTIFICATION
- NYS License and current registration as a Licensed Practical Nurse
- Valid New York State Driver’s License and maintain it in good standing
- CPR
