Job Qualifications

Education:

Current State of Colorado Licensed Practical Nurse (LPN)

Experience:

1. One year experience in a health care field or with adults with intellectual and developmental disabilities in a community, private or institutional setting preferred.

Other Requirements:

  • A criminal and Motor Vehicle record which meets company standards.

2. A valid Colorado Driver’s license.

3. Verified legal status to work in the United States.

4. Professional demeanor and ability to safeguard confidential information.

5. Excellent verbal and written communication skills and strong interpersonal skills.

6. Demonstrated ability in the areas of disability services, case management or quality assurance, preferred.

7. Advanced computer skills including Microsoft Office suite.

8. Ability to exercise judgment and initiate proper medical care for people supported.

Physical Demands/Working Conditions:

This position requires frequent walking, sitting and occasional standing and driving; occasional balancing, stooping, bending; frequent handling objects, and reaching with hands and arms; and constant finger movements. The position requires occasional lifting, carrying, pushing or pulling objects up to 25 lbs. The position also requires ordinary talking and hearing capability; full vision capabilities; and the ability to read/comprehend, write, communicate orally, and reason and analyze on a constant basis, and to perform calculations frequently. Work is performed using a computer and standard office machinery daily, and occasional use of a vehicle.

There are inherent risks in supporting people with intellectual and developmental disabilities. May be subject to name calling or other verbal insults in addition to physical aggression including, but not limited to, hitting, kicking, spitting, slapping, punching, throwing objects, etc. MVDS provides general and specific training to best prepare staff to prevent, and/or manage these occurrences.

Position Summary

The Licensed Practical Nurse (LPN) is responsible for monitoring, assessing, and addressing health and medical needs for individuals with intellectual and developmental disabilities under the supervisor of the Nurse Care Coordinator. The LPN will assist the Nurse Care Coordinator in overseeing the health and medical needs for Mountain Valley Developmental Services clients in the Garfield, Pitkin, Eagle and Lake Counties.

Essential Functions

Monitoring & Oversight

  • Visit people on site routinely and as needed to assess and determine medical needs and health.
  • Be on-call as requested by the Nurse Care Coordinator or management, to provide medical advice to staff in the event of a medical/medication question, treatment question, medical emergency or development of a serious new health problem to determine treatment.
  • Attend the IP and/or IDT meetings as needed for persons supported.
  • Complete an annual medical case review.
  • Monitor appointments and follow-ups to ensure continuous medical coverage for persons supported.
  • Review vitals monthly for persons supported and notify appropriate staff and/or physicians of any concerns.

Health and Medical

  • Exercise informed judgment and use individual competence and qualifications as criteria in seeking consultation, accepting responsibilities, and delegating nursing activity to others.
  • Perform ear lavages, blood draws, flu shots and other nursing tasks as needed, and as dictated by your license.
  • Develop and maintain all individualized healthcare, therapeutic and nutritional protocols.
  • Coordinate admissions and discharges to/from hospital or long term care settings, or hospice.
  • Coordinate and facilitate psychiatric services, including follow-up of medical recommendations, order changes, updating informed consents and obtaining dictation.
  • Monitor individuals taking psychotropic medication; conduct tardive dyskinesia screenings annually, monitor efficacy of medications and any possible side effects.
  • Attend or coordinate medical appointments and any follow-up, such as medical treatments and recommendations, and monitor implementation, as needed.
  • Obtain written orders and records from physicians and ensure they meet required standards. Obtain clarification on orders as needed.

Communication & Documentation

  • Serve as a liaison between medical professionals, pharmacies, legal guardians/families and staff to communicate needs related to health/medical issues of people supported.
  • Regularly attend staff meetings at group homes or day program sites to discuss the healthcare needs of the individuals supported at that site with the direct support staff.
  • Thoroughly document all health/medical related issues/correspondence regarding people supported.
  • Review all written health related communication and follow up as needed to assure timely healthcare for all individuals.
  • Obtain and maintain complete healthcare records including medical occurrence forms, dictations, test results, labs, orders, and medical histories.
  • Ensure medical communication is respectful, clear and informed.

Administrative

  • Establish, review, and revise written procedures related to health care to assure compliance with Rules and Regulations from oversight agencies.
  • Coordinate prescription drug disposal in accordance with Department of Health regulations on a regular basis.
  • Administer pre-employment TB tests.
  • Attend and participate in the Human Rights Committee (HRC).

Other Required Duties

Standards of Behavior/Conduct

  • Respond to all requests for information or communication in a timely manner.
  • Report all on-the-job injuries to immediate supervisor and Director of Human Resources within 24 hours of the occurrence and provide a detailed account of the events leading to the injury.
  • Respond as a Mandatory Reporter. Report all incidents of suspected client rights violations, mistreatment, abuse, neglect, or exploitation immediately according to Colorado state law and MVDS policies and procedures.

Attendance and Scheduling

  • Report for work on-time and as scheduled.
  • Enter daily, accurate timekeeping records in timekeeping database.
  • Attend all regular staff meetings and other meetings as requested by supervisory or administrative staff members.
  • Work schedule as assigned.
  • Get all time-off requests pre-approved.
  • Work at any MVDS facility and be responsible for own transportation to work.

Competencies

  • Complete all mandatory new hire requirements and training as identified by the State of Colorado and Mountain Valley’s training requirements within 90 days of hire and maintain courses and certifications as required.
  • Maintain skills with computers, take advantage of new technology, learn new tools quickly, use technology to enhance job performance and increase productivity. Maintain current understanding of technical processes/equipment.

Adherence to policies and procedures

  • Adhere to MVDS policies and procedures outlined in the policy manual, Employee Handbook, and other employee policies.
  • Complete other duties and responsibilities as assigned to ensure effective functioning of Mountain Valley’s programming.
